यह मंदिर बालोद जिला मुख्यालय से लगभग 15 किलोमीटर की दूरी पर स्थित गुजरा ग्राम में है यहां मंदिर आसपास के 12 गांव का एक विशेष पूजनीय एवं प्रतिष्ठित मंदिर है इसकी स्थापना गुजरा ग्राम के वासियों द्वारा किया गया है इस मंदिर पर भगवान पांडवों की मूर्ति की स्थापना की गई है इस कारण इसे पंडवान देव मंदिर कहा जाता है ऐसा मानना है कि पांडवों के वनवास काल पर पांचों भाई यहां पर निवास किए थे यहां पर विभिन्न प्रकार के पूजा अर्चना की जाती है जिसमें से एक प्रचलित पूजा है जब किसी वर्ष बारिश नहीं होती है तो यहां पर 12 गांव के प्रमुखों द्वारा एक विशेष पूजा अर्चना की जाती है लोगों की यह मान्यता है कि या पूजा करने से बारिश होती है कुछ और मान्यताओं के अनुसार जिनके संतान नहीं होते हैं वे आकर अपनी मन्नत मांगते हैं और यह भी देखा गया है कि उनकी मन्नते पूरी हुई है या स्थल जंगल के बीच में बसाया गया है जिससे यह जगह और भी पर्यटन की स्थल से रमणी हैं यहां पर आप हिरण और खरगोश आदि जीवो को वितरण करते हुए देख सकते हैं कुछ जंगल के नीचे जाने पर आपको तांदुला जलाशय का सुंदर नजारा देखने को मिल सकता है या मंदिर चैत्र और कुंवार नवरात्र पर पर रोशनी हो से जगमगा उठता है जिससे रात का नजारा और भी सुंदर और अलौकिक होता है यहां पर आप सब परिवार और दोस्तों के साथ आ सकते हैं
